- Cover letter outlining the major contributions of the nominee
- Biographic sketch
- 2–4 letters of support
- Up to 3 evidentiary materials
Award
- Award Recognition and Plaque
- Two nights hotel coverage at the conference hotel
- Up to $1000 in travel expense reimbursement
- One year membership to RSS
